The following four District Health Boards, in collaboration with a number of their respective primary care organisations, have agreed to participate in the Application of Self-management Systems Evaluation Trial (ASSET) Programme, namely:
·         Auckland DHB
·         Counties Manukau DHB
·         Tairawhiti DHB
·         Canterbury DHB
This Programme will implement a proof of concept integrated care trial for people with long term conditions using self management techniques, including approximately 200 telecare devices (50 allocated to each location), for a period of 1 year. The target group will be living in their place of residence with their principal care provider leading their clinical care in conjunction with other members of their integrated care team.   The Programme aims to assess any improvement in patient health and reduction in the cost of care by promoting an integrated care model which aims to reduce primary and secondary care attendances, and increase patients’ self management capability.
The Programme will be coordinated by a Core Team which comprises representatives from Auckland University’s commercial arm - UniServices, the School of Nursing, the National Institute for Health Innovation (NIHI) and the South Auckland Medical School.
